Only one sample request per baby per household is allowed with a total of 4 sample requests over a 2-year period.
Additional sample requests from same household for the same baby within 3 months will not be entertained.
Sample requests for the same baby using multiple addresses will not be entertained.

Incomplete entries will not be processed.
Please give us 4-6 weeks for delivery.
